Life Care Centers of America facilities opere a Skilled Nursing Facilities (SNFs) th provide:

  • Long term care for residents who require ongoing skilled support and compassione daily assistance
  • Short term, sub-acute rehabilition for pients recovering from surgery, illness, or injury
  • Collaborive care through an interdisciplinary team approach with nursing, therapy, and medical staff working together

Pient mix varies slightly by facility. As a therapy professional, you will help pients achieve functional progress and improve quality of life within a supportive, pient-centered environment.

Position Summary

The Transportion Coordinor/Driver loads, transports, and unloads pients for medical appointments and activity outings in accordance with all applicable laws, regulions, and Life Care standards.

Educion, Experience, and Licensure Requirements
  • High school gradue or equivalent
  • Valid drivers license or valid commercial drivers license in current Ste, as applicable, with a sisfactory driving record per Life Care standards
  • Prior experience in health care or gerirics preferred
Specific Job Requirements
  • Complete pre inspections before use of vehicle
  • Monitor regular maintenance (oil changes, tire rotions, etc.)
  • Make independent decisions when circumstances warrant such action
  • Knowledgeable of practices and procedures as well as the laws, regulions, and guidelines governing functions in the post acute care facility
  • Implement and interpret the programs, goals, objectives, policies, and procedures of the department
  • Perform proficiently in all competency areas including but not limited to: pient rights, and safety and sanition
  • Maintains professional working relionships with all associes, vendors, etc.
  • Maintains confidentiality of all proprietary and/or confidential informion
  • Understand and follow company policies including harassment and compliance procedures
  • Displays integrity and professionalism by adhering to Life Cares Code of Conduct and completes mandory Code of Conduct and other approprie compliance training

  • Safely load, transport, and unload pients

  • Assist in the evacuion of pients

  • Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
